Appointment requests, patient registration, and consent forms are completed through our secure patient portal, which is operated by our practice-management provider and protected under HIPAA. If you call or email us, we use what you share only to respond to you and to provide care. Please don't send medical details or other sensitive information by email; use the secure patient portal instead.
